I know someone who’s company closed down , he was there for over 10 years and in that time they have been certified about 2 times.
The company never physically gave any of the employees the certificates.
When the company closed he left without thinking about getting the actual certificate.
How can he get it a copy of his certificate ?
Would his W2 Stubs prove of forklift certification ?
Can you get it from OSHA ?
How long are they valid for ?
If it expires , why would a company ask for a certificate if they will just have to re-certify their employee’s ?
And there is a government agency that issues licenses (OSHA)

So the certificate has to be like a "drivers license" .
Forklift certificates are basically company specific if the training is internal. It is OSHA required, by not OSHA accredited. Very few companies accept training from other companies. It is not smart to do for a company since they don’t know the training was done correctly and it was not specific to their company.
If you took the training from an outside training company, they can provide it, but if they close down, you may be out of luck. OSHA is not given copies so they will be no help.

Each company does their own licensing. It’s not like having a license to drive a car. There is no state or federal organization that issues fork lift licenses. He can put his past work experience on his resume and prospective employers can verify it if they choose to.
Trust me – I have over 20 years experience as a fork lift operator and 10 years experience as a fork lift trainer and tester. OSHA does not "issue" fork lift licenses. They may require people complete OSHA certified training and have a valid company issued license, but they have nothing at all to do with the licensing process.
